About The Position

The Pharmacist performs pharmaceutical care activities for customers/patients within the service area. This role involves monitoring, documenting, and reporting patient outcomes, as well as providing or coordinating the dispensing of medications, home medical equipment, and ancillary supplies. The Pharmacist also participates in an on-call schedule.

Requirements

  • Bachelor of Science in Pharmacy
  • Minimum of two (2) years of recent experience in an acute-care setting or one (1) year of recent experience in home infusion therapy
  • Experience with compounding infused drugs
  • Registered Pharmacist License in state(s) of practice
  • Willing to obtain and maintain compliance with additional state licensure outside of home state, to support company operation
  • Good organizational and time-management skills
  • Excellent verbal and written communication skills
  • Schedule flexibility

Nice To Haves

  • Doctor of Pharmacy preferred
  • Participation and membership in professional societies and organizations is desirable

Responsibilities

  • Evaluates and monitors drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses, and other healthcare professionals.
  • Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice.
  • Adheres to all appropriate standards of accreditation and participates in quality assurance activities.
  • Coordinates and facilitates new referrals and admissions through collaboration with nurses, customer service, and distribution.
  • Reviews, receives, and verifies all new patient orders and documents on the respective order form.
  • Assists the physician and other involved healthcare professionals with planning and implementing patient care in the home, setting therapeutic objectives for drug therapy, establishing parameters for monitoring the safety and efficacy of drug therapy, and preparing regular progress reports.
  • Conducts and documents initial and ongoing assessment of specific patient, drug, and disease information to identify real and potential drug-related problems or needs.
  • Monitors the outcomes of drug therapy by collecting, evaluating, and interpreting the results of laboratory data, clinical data, and other pertinent patient information.
  • Provides consultation and answers inquiries from patients, families, physicians, and other healthcare professionals regarding drugs and their use.
  • Maintains thorough and appropriate documentation in the customer/patient record of all clinical activities and communication with patients, physicians, and other healthcare professionals.
  • Processes and checks all medications, supplies, and equipment prior to dispensing from the pharmacy.
  • Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars, and conferences.
  • Pharmacists’ duties are extended to include overseeing of the Pharmacy technician functions as required by Pharmacy Board rules.
