About The Position

This is a full-time, day shift position at Pickerington Methodist Hospital, with a focus of 80% Medication Safety and 20% Operational tasks. Shift hours are 7:00-15:30, with no holidays but participation in weekend rotations. The role involves core pharmacist responsibilities such as dispensing and compounding medication, patient counseling, interpreting prescriptions, managing patient drug therapy, and performing drug utilization. Pharmacists are expected to work efficiently both independently and as part of a team, demonstrating critical thinking, problem-solving skills, and the ability to multitask. Strong interpersonal skills, including effective mentorship, clear communication, collaboration, empathy, and the ability to build trust, are essential. The position requires collaboration with physicians, nurses, and other healthcare practitioners to ensure safe, high-quality, and cost-effective drug therapy. Pharmacists may administer medications as per state law and practice setting and will serve as preceptors for pharmacy students and residents. The role demands in-depth conceptual and practical knowledge in the job discipline, basic knowledge of related disciplines, and the ability to solve complex problems independently with minimal guidance. The individual may lead projects or project steps and acts as a resource for less experienced colleagues, explaining difficult or sensitive information and building consensus. Requirements

  • Bachelor of Science or Doctor of Pharmacy degree from an accredited institution
  • Current Ohio pharmacist license (OPL - Ohio Pharmacists License - Ohio Board of Pharmacy)
  • Basic Life Support Certification (current or obtained during orientation period, unless greater than 50% of scheduled work hours are completed off-site or not in a patient care setting)
  • ASHP accredited residency or equivalent pharmacy practice experience
  • Completion of a Pharmacy Practice residency or equivalent hospital experience
  • Pharmacokinetic drug dosing experience
  • Drug therapy management

Nice To Haves

  • Pharmacy Board Certifications or other certifications
  • Advanced-level degree in healthcare field
  • Collaborative practice experience
  • Advanced Cardiac Life Support Certification (may be required to obtain as determined by manager)

Responsibilities

  • Demonstrate consistent behaviors aligned with high reliability organization principles to provide the safest and most effective care.
  • Review provider orders for correct drug choice, dosage form, dosing schedule, and potential drug-drug interactions or allergies, contacting providers for clarification as appropriate.
  • Dispense and compound medications and drug therapy-related devices in accordance with standards of practice and OhioHealth pharmacy policies, ensuring medication integrity, proper labeling, and compatibility.
  • Interpret prescriptions and prepare individualized care plans for patients to ensure optimal therapeutic outcomes and maximize compliance with therapy instructions.
  • Manage individuals’ drug therapy by initiating, monitoring, and modifying prescriptions in collaboration with a provider.
  • Provide drug information to providers, other healthcare professionals, and patients as needed.
  • Counsel individuals regarding their drug therapy, including recommending drug therapy-related devices, assisting in the selection of non-prescription drugs, and providing instruction in their proper use.
  • Provide associate and learner development and training, including precepting pharmacy students and residents, and delivering educational programs for pharmacy, nursing, and medical staff.
  • Direct and guide pharmacy technicians and pharmacy support personnel.
  • Partner with individuals (patients, caregivers, and other healthcare professionals) to prevent, manage, and/or slow disease progression through patient advocacy, wellness and health promotion, triage and referral, patient education, and self-management.
  • Assist with guideline/policy development and implementation, and participate in performance improvement initiatives for practice advancement.
  • Ensure proper billing procedures.
  • Lead safety, stewardship, and utilization strategies for designated site(s) in collaboration with the system team.
  • Facilitate and manage activities of various site and system committees and clinical guidance councils.
  • Develop, enforce, and maintain policies and procedures that promote and enhance patient outcomes through safe, effective, cost-efficient, and appropriate medication therapy use.
  • Utilize data, published best practice standards, and published events or recommendations to identify opportunities for proactive change in the medication-use system to improve patient outcomes.
  • Integrate medication improvement initiatives, activities, and changes with clinical and operational department programs and committees.
  • Ensure compliance with evidence-based recommendations, national patient safety recommendations, and regulatory standards related to safe and appropriate medication use.
