As a Liver Transplant Pharmacist at Cedars-Sinai Medical Center in Los Angeles, California will maintain the quality and support of highly immunocompromised patients requiring high level of specialized services. The Liver Transplant Pharmacist will help manage nephrology consults, pre-transplant infusions for highly sensitized patients, post-transplant infusion patients , and cover in-patient services including managing non-formulary requests. The Pharmacist will be responsible for entering therapy plans, conduct chart reviews, compose letters of medical necessity as well as participate in peer-to-peer reviews for denied insurance claims. The Pharmacist will also participate with in-patient rounding which would include discharge teachings, medication reconciliation, ordering and coordinating last minute medication changes prior to discharge.

Requirements

  • Must be a licensed Pharmacist.

Responsibilities

  • Maintain the quality and support of highly immunocompromised patients requiring high level of specialized services.
  • Manage nephrology consults.
  • Manage pre-transplant infusions for highly sensitized patients.
  • Manage post-transplant infusion patients.
  • Cover in-patient services including managing non-formulary requests.
