Requirements

Current New York State Pharmacist registration.
Accredited continuing education as required/defined by New York State
Bachelor’s degree in pharmacy or Pharm. D from accredited Pharmacy program required
Demonstrates the knowledge & skills necessary to provide care appropriate to the age of the patient.
Possessing an in-depth knowledge of medications and their uses.

Nice To Haves

Board Certification strongly preferred
Minimum of one year of ambulatory care pharmacy experience preferred

Responsibilities

Participates in programs and procedures designed to promote the appropriate, safe and effective use of medications for Thompson Health patients and residents.
Provides professional services and support in the following distributive and clinical areas: Medication distribution and compounding, Drug Information, Education and Resource Materials, Formulary Management/Drug Usage, Adverse Drug Reactions, Medication Error Prevention, Nursing/Pharmacy Liaison, Clinical Computer Applications, Medication Use Evaluation, Program Development and Implementation.
Demonstrates skill in daily communication including effectively gathering and disseminating information.
Exhibits flexibility in decision-making and prioritizing assignments.
Proactively resolves problems, makes recommendations, and implements continuous quality improvements.
Manages daily workload expectations.
Maintains and enhances knowledge of all accrediting and/or regulatory agency standards (i.e. Joint Commission, DOH, etc.).
Actively guards the confidentiality of sensitive information including but not limited to the patients, staff and the health system.
Provides care appropriate to the age of the patient.
Lives the CARES values at all times.
