Requirements

  • Education pursuant to licensure requirement.
  • 3 Years experience in a hospital pharmacy practice; Or 1 Year completion of PGY1 Pharmacy Residency.
  • California Pharmacist License - CA State Board of Pharmacy -REQUIRED
  • ACLS Certification (Advanced Cardiac Life Support) - American Heart Association -REQUIRED (within 90 days of hire)
  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED (within 90 days of hire)

Responsibilities

  • Review all medication orders for appropriate dose, duration, frequency, dosage form and indication.
  • Monitor therapy for potential or actual drug interactions (e.g., drug-drug, drug-disease, drug-allergy, etc.).
  • Ensure compliance with inpatient drug formulary.
  • Provide counseling to patients on medication use consistent with professional standards, including first dose teaching, food-drug interaction counseling, discharge counseling, medication classes, and compliance aids.
  • Assess patient understanding of medication regimen and provide tailored education.
  • Participate in admission and discharge medication reconciliation, including documentation of activities as needed.
  • Ensure drug regimen is consistent with Sharp medication use guidelines and policies (e.g., antimicrobial use, IV to oral, renal dosing, serum drug concentration protocols).
  • Adjust regimens based on approved protocols and consultation with the interdisciplinary team.
  • Complete a daily profile review for each patient, assessing various factors like age, sex, height, weight, renal/hepatic function, pregnancy/lactation status, appropriateness of medication parameters, lab values, microbiology results, antimicrobial therapy, potential interactions, adherence to guidelines, therapeutic drug monitoring, adverse drug reactions, and disease progression.
  • Communicate necessary information to pertinent health care providers, patients, and caregivers, and encourage health promotion and prevention.
  • Assess the patient’s progression to achieving desired medication treatment goals.
  • Demonstrate knowledge and competency of chemotherapy dosing, monitoring, and established guidelines (if applicable).
  • Participate qualitatively in the clinical intervention program through consistent issue identification, provision of reliable best practice/literature-based recommendations, and follow-through with prescribers and other clinicians.
  • Perform quantitative work by completing and documenting interventions consistent with established averages for the pharmacy department/shift.
  • Communicate with nurses and physicians in a timely manner about therapeutic issues or problems.
  • Follow up on formulary compliance issues and/or communicate issues to management.
  • Document interventions, patient contact, and multidisciplinary communication consistent with Sharp policies and procedures.
  • Participate in scheduled rounds in their care area (when applicable).
  • Oversee Pharmacy Resident and/or PharmD student participation on rounds.
  • Work collaboratively with the interdisciplinary team to maximize individual patient outcomes.
  • Address patient specific drug information questions and document requests promptly and professionally.
  • Prepare and dispense medications accurately and in a timely manner in accordance with recognized standards of practice, legal and regulatory requirements, and hospital pharmacy policies and procedures.
  • Assure controlled substances are dispensed with accuracy and maintain integrity of all record keeping, assisting with narcotic inventories.
  • Demonstrate proficiency with and utilization of automation and advances in technology (EMR System, Automated Dispensing Cabinet system (Pyxis), IV Workflow Management system (DoseEdge), physician order entry, etc.).
  • Demonstrate knowledge of age specific medication delivery systems (e.g., NICU admixtures, pediatric oral syringes, adult TPN / TNA solutions, final admixture concentration etc.).
  • Demonstrate knowledge and competency of IV Admixture / Aseptic Technique including the standards of USP 797, 800 and 795.
  • Demonstrate knowledge and competency of chemotherapy preparation (as applicable).
  • Participate in Code-Blue emergencies as required, mix and label medications as needed, and provide information (within entities that provide medical emergency response).
  • Understand investigational drug protocols and procedures for investigational drug research studies, implementation, administration, and record keeping (as applicable).
  • Participate in cost management initiatives including managing appropriate inventory levels, reducing waste, and completing borrow/loan paperwork.
  • Oversee pharmacy operations and provide direction to Pharmacist Residents, Pharmacist Interns, Pharmacy Technicians, and other non-licensed personnel.
  • Actively participate in concurrent medication safety event (MSE) monitoring programs, demonstrated by interventions and RL submissions.
  • Perform initial analysis of MSE's and assist in the research and identification of causative factors.
  • Perform non-voluntary monitoring (i.e., trigger reports, harm monitors, alerting orders).
  • Participate in ongoing data collection for medication use evaluations (MUE) as assigned.
  • Complete monthly assigned nursing unit / drug storage area inspections on time, including paperwork, documenting discrepancies, and making recommendations.
  • Bring critical issues to the attention of management.
  • Ensure security of medication storage and compliance with Medication Management (MM) standards.
  • Assure compliance with hospital, integrated health system and department policies and procedures.
  • Monitor compliance with state and federal laws and regulations related to the practice of pharmacy as well as standards and regulations for other regulatory agencies.
  • Participate in assigned department, hospital, and health-system committees or projects.
  • Answer phone in a timely, friendly manner, stating department, name, and title.
  • Foster positive relationships through consistent, respectful, professional interactions with customers.
  • Respond to requests in a timely, caring manner consistent with Sharp behavior standards.
  • Clearly communicate goals and instructions to other staff members.
  • Provide relevant handoffs between shifts (written/oral).
  • Effectively intercept and troubleshoot problems, demonstrating problem solving, prioritization, and communication skills.
  • Apply appropriate policies and procedures, including Chain of Command escalation, when faced with complex situations.
  • Effectively evaluate ongoing workflow issues and direct daily workload to achieve efficiency and maximize resource utilization.
  • Attend and participate in staff meetings or review staff meeting communications.
  • Read work related email at a minimum on assigned workdays, responding professionally and in a reasonable timeframe.
  • Actively engage in personal continuing education and professional development.
  • Orient and train new pharmacy staff as assigned.
  • Provide direction and precept pharmacist residents and interns as assigned.
