About The Position

Due to growth, we are adding a Pharmacist II, Clinical Operations to our team. The Pharmacist II, Clinical Operations will be responsible for performing telephonic consultations with patients regarding the use of their specialty medication and management of their disease state. They will perform medication reconciliation, screen for potential drug interactions, and provide side effect management recommendations. Another main duty of this role will include a high volume of product verification to ensure accuracy of orders prior to shipment. The Pharmacist II, Clinical Operations can expect to interpret prescribers’ prescriptions, contact prescribers and/or patients to verify information on prescriptions, and expedite the processing of orders. The individual in this position will be expected to meet department production metrics and service levels, support quality standards, resolve patient issues, and vary work priorities and activities to accommodate business needs. The Pharmacist II, Clinical Operations is expected to train less experienced pharmacists, mentor pharmacy experiential learners (interns/students/residents), and act as a knowledgeable resource for the Clinical Operations team regarding complex workflow processes and problem solving. The individual in this position should have extensive clinical knowledge of specialty pharmacy disease states and medications, be able to solve complex problems by taking a new perspective using existing solutions, work independently with minimal guidance, and represent the organization using patient-centered care to enhance the patient experience.

Requirements

Graduate of ACPE Accreditation School of Pharmacy with a B.S. Pharmacy or PharmD degree
Current, unrestricted Pharmacist license in state where any on-site pharmacy work will be conducted; must continually maintain education credits to maintain active licensure.
Job specific certification (such as CSP or BCOP) required within 6-12 months of hire.
Minimum of 2 years as a licensed Pharmacist required.
Specialty pharmacy experience or solid understanding of specialty pharmacy drugs including REMS and Limited Distribution Products best practices required.
Internal applicants: meet and/or exceed all department productivity metrics, service levels, and quality standards expected of Clinical Operations Pharmacist.
Participate in, adhere to, and support compliance program objectives.
The ability to consistently interact cooperatively and respectfully with other employees.

Responsibilities

Verify prescriptions for pharmaceutical preparations and devices for patients at company facilities; maintain accurate and detailed records of all prescription transactions, and submit appropriate claims for third-party reimbursement; consult with physicians and nurses regarding pharmaceutical questions; advise physicians on formulary medications and cost-effective and innovative therapies; answer pharmaceutical questions from patients and staff; monitor storage, distribution, and use of pharmaceuticals.
Provide clear, detailed clinical documentation of patient consultations and interventions in pharmacy software in compliance with internal policies and procedures and external regulatory bodies (such as URAC) and/or EMR systems efficiently.
Provide oversight and quality assurance to pharmacy technicians.
Interpret and clinically evaluate prescriptions for medications within all specialty disease states (including oncology and fertility) based on available information and resources. Follow through on complex orders. Contact providers and/or patients to verify information related to prescriptions such as drug strength, intended directions, and diagnosis to expedite processing of orders. Verify and confirm validity of controlled substances.
Contact providers for new prescriptions, consult with prescribers and nurses regarding clinical questions, and contact pharmacies for prescription transfers. Work with providers to convert prescriptions to generic, biosimilar, or preferred drugs whenever possible.
Verify prescription information entered in the system by data entry or order entry. Answer clinical questions from the pharmacy team. Provide inter- and intra-departmental feedback and coaching on appropriate prescription entry and documentation as necessary.
Train less experienced pharmacists and mentor pharmacy experiential learners (inters/students/residents). Act as a knowledgeable resource for the Clinical Operations team for complex workflow processes and appropriate problem solving. Assist with complex clinical calls. Utilize extensive clinical knowledge of specialty pharmacy disease states and pharmacy processes to assist in workflow for all disease states as well as in unique workflows including manufacturer programs. Efficiently identify resources and appropriately triage situations to expedite workflow.
Quickly adapt to new workflows and processes and serve as a positive role model to the team in support of change management. Solve unique pharmaceutical problems. Follow through on patient care activities. Stay current with advancements in specialty pharmacy therapies, including new drug approvals and specialty disease state treatment guidelines.
Provide patient education through telephonic consultations for patients who are part of all lines of business and IDN pharmacies in accordance with Lumicera’s Patient Management Program. The consultations will focus on effective assessment and communication of disease state management, side effect mitigation strategies, administration of medication, appropriate storage, and therapeutic appropriateness, while following a patient-centered care model. Answer clinical questions from patients and caregivers.
Document and report medication errors in accordance with company policies and manufacturer contracts. Facilitate error reduction and prevention methods based off errors and ISMP standards. Adhere to all Lumicera pharmacovigilance standards.
