Our Consultant Pharmacists are dedicated medication experts who make a meaningful impact through medication optimization, helping residents live their best lives. They lead interdisciplinary care teams while serving as a trusted clinical resource for prescribers and nursing staff. They also implement cost-containment strategies that reduce medication spend and improve nursing efficiency, and deliver high-quality medication management and clinical support that helps facilities optimize care, reduce regulatory risk, and adapt to an evolving healthcare landscape. The Pharmacist Consultant is a hybrid position responsible for supporting skilled nursing and long-term care facilities throughout the Central Valley Region. Qualified candidates must reside within the designated territory. This role requires monthly travel to assigned skilled nursing facilities to conduct on-site clinical consulting activities, including audits, regulatory inspections, medication regimen reviews, and controlled substance drug destruction as needed. Additional travel within the territory may be required to support client needs and ensure regulatory compliance.

Requirements

Active Pharmacist license in good standing.
Immunization Certification through an accredited organization (such as APhA) is required.
Ability to travel throughout the assigned territory on a monthly basis.
Strong knowledge of long-term care pharmacy regulations, medication management, and consultant pharmacy practices.
Bachelor’s degree from an accredited School of Pharmacy or Pharmacy Doctorate
Current license required.
Must be able to work weekends or outside of normal business hours when needed
Ability to lift patient charts repetitively
Ability to stand for long periods of time
Good customer service skills
Ability to learn and understand state and federal regulations for long-term care

Nice To Haves

Dual license preferred
One-year experience in acute, long-term care or home health preferred
Consultant or Clinical Pharmacist experience preferred
IV clinical experience preferred
Understand of formulary management

Responsibilities

Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability. Makes recommendations for therapy changes to the physician and facility staff as appropriate
Reviews the drug regimen and clinical indicators of each resident as assigned at least monthly and reports any irregularities to the Director of Nursing, Administrator and the attending physicians with a written report of findings and recommendations
Documents all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members
Maintains knowledge of pharmacy laws and federal and state regulations concerning long-term care
Works cooperatively as a member of a team
Maintains knowledge of pharmaceutical care practices for long term care residents
