Requirements
- Active and unrestricted California Pharmacist (RPh) license required
- Doctor of Pharmacy (PharmD) degree from an accredited institution preferred
- Prior experience in long-term care (LTC) or consultant pharmacy strongly preferred
- Familiarity with CMS regulations, OBRA guidelines, and skilled nursing facility (SNF) compliance standards
- Strong clinical assessment and medication therapy management (MTM) skills
- Excellent communication and interpersonal skills for working with multidisciplinary teams
- Ability to manage multiple facility accounts and work independently in the field
Nice To Haves
- Board Certified Geriatric Pharmacist (BCGP) designation is a plus
Responsibilities
- Conduct monthly medication regimen reviews (MRRs) for long-term care facility residents
- Collaborate with physicians, nursing staff, and interdisciplinary care teams to optimize drug therapy outcomes
- Identify and resolve drug-related problems, including interactions, duplications, and inappropriate dosing
- Ensure facility compliance with federal and state pharmacy regulations, including CMS guidelines
- Provide staff education and in-service training on medication safety and best practices
- Assist in developing and updating pharmacy policies and procedures for LTC facilities
- Support antibiotic stewardship and quality improvement initiatives within partner facilities
