About The Position

The position serves as a provider with prescribing privileges as outlined in the scope of practice for the Clinical Pharmacy Specialist (CPS) in Pain and to provide comprehensive medication therapy management. The responsibility of the CPS is the provision of pharmaceutical care through direct care with patients and consultation with providers. The CPS functions as part of the Pain teams and is assigned patients to support continuity of care and ensure the best use of pharmaceuticals.

Requirements

  • Must be a U.S. Citizen.
  • Must be proficient in written and spoken English.
  • Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
  • Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
  • Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
  • Expert understanding of regulatory and quality standards for their program area.
  • Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
  • Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
  • Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
  • Maintains licensure and BCLS certification.
  • Completes required professional and organizational education.

  • PGY1 Pharmacy Residency
  • PGY2 Pharmacy Residency in Pain/Palliative Care or PGY1 + 3 years >50% clinical experience in Pain
  • BCPMP, BCPP, or BCPS certification
  • Clinical Pharmacy Specialist (Pain) - Direct patient care with scope of practice
  • DEA license or experience prescribing/managing controlled substances

Responsibilities

  • Designs, monitors, and adjusts therapeutic drug plans.
  • Reviews medication regimens for safety, effectiveness, and interactions.
  • Interprets labs and applies pharmacokinetic/pharmacodynamic principles to dosing.
  • Provides tele-health assessments and manages remote-site emergencies.
  • Documents interventions, obtains medication histories, and conducts drug use evaluations.
  • Optimizes drug therapy, manages adverse events, and coordinates follow-up care.
  • Reviews prior authorizations; ensures compliance with drug therapy policies and initiatives.
  • Participates in committees and organizational clinical activities.
  • Trains staff, students, residents, and patients.
  • Contributes to service development, research, and new employee onboarding.
  • Serves as preceptor; supports residency accreditation and professional outreach.
  • Acts as a drug information expert; maintains current therapeutic knowledge.
  • Provides accurate, timely responses using reliable resources.
  • Follows safety protocols, uses required PPE, and reports hazards or incidents promptly.
  • Provides team guidance; serves as liaison across departments.
  • Identifies opportunities to enhance pharmacy services and communicates them to leadership.
  • Represents Pharmacy Services professionally.
  • Supports compliance with TJC, OIG, and other regulatory standards.
  • Maintains licensure and BCLS certification.
  • Completes required professional and organizational education.
  • Performs additional duties necessary for patient care.
