Under the supervision of the Manager of Clinical Pharmacy Services and the Clinical Pharmacy Coordinator the Pharmacist is responsible to ensure safe and effective drug therapy optimizations and management for hemodialysis patients. The Renal Pharmacist participates in all aspects of care for renal patients including immunizations, prescribing/deprescribing, pharmacokinetic monitoring, and is a steward for all appropriate medication use. The candidate will also engage in quality committees, quality improvement and research initiatives. Further, the candidate will provide educational sessions to other health care professionals, as well as precept pharmacy learners.

Requirements

  • Bachelor of Science in Pharmacy or an entry level Pharmacy Doctor of Pharmacy from a CCAPP (or similar standard) accredited university required
  • Eligible for registration with the Nova Scotia Pharmacy Regulator (NSPR)
  • BLS (Basic Life Support) required
  • Strong knowledge of drugs and therapeutics
  • Effective oral and written communication skills
  • Ability to make accurate and timely recommendations regarding safe and cost-effective use of drugs

Nice To Haves

  • Completion of a Pharmacy Residency Program preferred
  • Minimum two years experience in clinical practice in a hospital pharmacy setting preferred
  • Competencies in other languages an asset, French preferred
