About The Position

This is a full-time (32 hours/week), variable shift position with a Clinical/Operations focus at Berger Hospital. The majority of shifts are days (0700-1530), with the rest being evenings (1230-2100). There are some variances to this schedule when considering weekend rotations and occasional night shift coverage. Weekends are typically every 4th-5th week. The position requires in-depth conceptual and practical knowledge in the job discipline and basic knowledge of related job disciplines. It involves solving complex problems, working independently with minimal guidance, and potentially leading projects or ongoing activities. The role acts as a resource for colleagues with less experience. The pharmacist collaborates with physicians, nurses, and other health-care practitioners to provide safe, high-quality, and cost-effective drug therapy. Pharmacists may administer medications as applicable by state law and practice setting. They will also serve as preceptors for pharmacy students and pharmacy residents. Responsibilities require focus on safety, quality, and efficiency in performing job duties as part of a High Reliability Organization (HRO).

Requirements

Bachelor of Science or Doctor of Pharmacy degree from an accredited institution
Current Ohio pharmacist license
Basic Life Support Certification—current or obtained during the orientation period
ASHP accredited residency or equivalent pharmacy practice experience
Pharmacokinetic drug dosing experience

Advanced Cardiac Life Support Certification
BS or Doctor of Pharmacy degree from an accredited institution and a current Ohio pharmacist license. Completion of a Pharmacy Practice residency or equivalent hospital experience.

Responsibilities

Reviews provider orders for correct drug choice, dosage form, dosing schedule, and possible drug-drug interactions or allergies. Contacts providers to clarify orders as appropriate.
Dispenses and compounds medications and dispenses drug therapy-related devices in accordance with standards of practice and OhioHealth pharmacy policies.
Ensures all medications, chemicals and supplies are dispensed without suboptimal integrity.
Ensures appropriate labeling for proper administration of medication.
Interprets prescriptions and determines compatibility in accordance with standards of practice.
Prepares care plan for patients to ensure optimal therapeutic outcomes and maximize compliance with therapy instructions.
Manages individuals’ drug therapy by initiating, monitoring, and modifying prescriptions issued to patients in a consulting relationship with a provider.
Provides drug information to providers, other health care professionals, and patients as needed.
Counsels individuals regarding their drug therapy, including recommending drug therapy-related devices, assisting in the selection of non-prescription drugs for the treatment of common diseases and injuries, and providing instruction in their proper use.
Provides associate and learner development and training, including precepting students and residents.
Provides educational programs for pharmacy, nursing, and medical staff as needed.
Directs and guides pharmacy technicians and pharmacy support personnel.
Manages individuals’ drug therapy by initiating, monitoring, and modifying prescriptions in collaboration with a provider.
Partners with individuals (patients, caregivers, and other health care professionals) to prevent, manage, and/or slow progression of disease through patient advocacy, wellness and health promotion, triage and referral, patient education, and self-management.
Assist with guideline/policy development and implementation as well as participate in performance improvement initiatives, including those for practice advancement.
Ensures proper billing procedures.
Leads safety, stewardship, and utilization strategies for designated site(s) in collaboration with system team.
Facilitates and manages activities of various site and system committees and clinical guidance councils.
Develops, enforces, and maintains policies and procedures that promote and enhance patient outcomes through safe, effective, cost-efficient, and appropriate medication therapy use.
Utilizing data, published best practice standards, and published events or recommendations, identifies opportunities for proactive change in the medication-use system to improve patient outcomes.
Integrates medication improvement initiatives, activities, and changes with clinical and operational department programs and committees.
Responsible for ensuring compliance with evidence-based recommendations, national patient safety recommendations, and regulatory standards related to safe and appropriate medication use.
