Responsible and accountable for providing safe, effective, and prompt medication therapy to hematology/oncology patients. Displays an understanding of diversity. Collaborates with multidisciplinary healthcare teams to optimize patient care’s pharmacotherapeutic aspects via various intra-department assignments; provides support in delivering optimal medication therapy to hematology/oncology patients. Develops and/or maintains an active clinical pharmacy practice on the assigned service(s); participates actively in rounds and/or outpatient clinic coverage (including assigned on-call and weekend coverage). Assesses and evaluates medical and drug literature critically; develops/leads formulary management, medication therapy recommendations, and optimization initiatives. Participates in quality assurance (QA) activities (e.g., drug utilization evaluations, quality improvement (QI) initiatives, and evaluating clinical pharmacists’ impact on drug therapy measurements). Establishes and/or maintains a viable practice suitable for educating and training. Performs Pharmacist duties (as required). Performs other duties.

Requirements

Doctor of Pharmacy (Pharm.D.) degree from an Accreditation Council for Pharmacy Education (ACPE) accredited school of pharmacy
One (1) year of relevant registered pharmacist work experience
Licensed [or eligible for licensure (i.e., meets all statutory requirements for licensure application)] as a pharmacist in accordance with Florida Statutes (F.S.), Title XXXII (Regulations of Professions and Occupations), Chapter 465 (Pharmacy), Sections 007 through 013.
Basic Life Support (BLS) certification from an American Heart Association (AHA) or American Red Cross (ARC) recognized provider.

Nice To Haves

Post-Graduate Year 1 (PGY1) Pharmacy Residency completion.
Post-Graduate Year 2 (PGY2) Pharmacy Residency (in specialization area) completion.
Two (2) to four (4) years of clinical pharmacy experience.
Board of Pharmacy Specialties (BPS) Board Pharmacy Specialty Certification (BCOP) and/or BPS Board Certified Pharmacotherapy Specialist (BCPS) credential(s).

Responsibilities
