About The Position

This is a part-time Pharmacist position for St. Mary's Hospital in Madison working a total of 24 hours per week. Work Schedule Variable shifts, including a mix of AM and PM hours. Hours range from 0630 to 2300. Every 3rd weekend rotation. No on-call requirement. A one-time sign on bonus (less taxes) may be available to qualified individuals with a 2 year service commitment. Provides pharmaceutical care to patients, verifies medication orders, compounds and dispenses pharmaceuticals in accordance with federal, state and local regulations and hospital policy. Serves as a resource to other healthcare professionals including pharmacy students and residents and ensures that patient care meets standards of safety and effectiveness. Oversees the duties of the pharmacy technicians and interns.

Requirements

• Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from accredited school of pharmacy
• No experience required
• Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment.
• State of Work Location: Illinois Pharmacist, Registered - Illinois Department of Financial and Professional Regulation (IDFPR)

Responsibilities

• Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable.
• Documents medication related plans and interventions in electronic medical record.
• Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners.
• Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.
• Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients. This includes supervision of pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system.
• Precept pharmacy students and residents, as applicable.
• Works in a constant state of alertness and safe manner.
• Performs other duties as assigned.
