About The Position

Provides pharmaceutical care to patients, verifies medication orders, compounds and dispenses pharmaceuticals in accordance with federal, state and local regulations and hospital policy. Serves as a resource to other healthcare professionals including pharmacy students and residents and ensures that patient care meets standards of safety and effectiveness. Oversees the duties of the pharmacy technicians and interns. This role primarily oversees sterile compounding activities and regulatory compliance with some staffing in the central pharmacy, averaging about 80% sterile compounding and 20% staffing.

Requirements

  • Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from accredited school of pharmacy
  • Inpatient hospital pharmacy experience
  • Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment.
  • State of Missouri Pharmacist license

Responsibilities

  • Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable.
  • Documents medication related plans and interventions in electronic medical record.
  • Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners.
  • Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.
  • Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients.
  • Supervises pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system.
  • Precept pharmacy students and residents, as applicable.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.
