Pharmacist - Clinical Inpatient Pharmacy
About The Position
The purpose of this position is to be responsible for the filling and dispensing of medication orders, including the proper handling, usage, and storage of drugs in the pharmacy. The role maintains departmental standards and translates policies and procedures into daily pharmacy operations. The pharmacist acts as an information resource for patients, physicians, nurses, and other healthcare professionals regarding medication therapy and drug-related information. The Emergency Medicine Pharmacy Team consists of five pharmacists providing 24-hour coverage, responsible for order verification in the ED and responding to critical patients (trauma, sepsis, sedation, stroke activations). Pharmacists also manage culture callbacks and teach medical and pharmacy learners. While the primary focus is Emergency Medicine, there are opportunities to work in other hospital areas.
Requirements
- Graduation from college with a degree in Pharmacy
- Possession of a valid license to practice pharmacy in the State of Minnesota
- Excellent problem-solving and communication skills (verbal & written)
- Must be a self-starter, able to work under pressure and be flexible in setting priorities
- Ability to do accurate, detailed analysis of complex information and issues is essential
- Possess extremely strong customer service skills
- Effectively interacts with team members and other departments
- Adaptable to changing priorities, tasks, and work schedules to meet customer service standards and needs
Nice To Haves
- Board certification in emergency medicine or critical care or advance credentials in area of practice
- ASHP PGY-2 in emergency medicine or critical care, (or completing residency) OR minimum of five years of experience as an emergency medicine pharmacist
Responsibilities
- Verifies, prepares, compounds and accurately dispenses medications
- Interprets physician medication orders for accuracy and appropriateness, informing them of any errors, omissions, or discrepancies and suggesting appropriate or alternate drug therapy based on patient condition
- Maintains accurate and proper patient medication profiles, monitoring for the correct drug, dosage form, strength, length of therapy, and interaction potential
- Monitors patient reactions to drug therapy
- Serves as an information resource person to medical personnel
- Counsels patients about their prescriptions
- Maintains a working knowledge of, and conforms to, legal requirements, department policies and procedures concerning drug distribution, and stays updated on recent developments in pharmacy
- Assumes 'charge pharmacist' responsibilities as required, coordinating pharmacists and supportive personnel, assigning work, delegating tasks and responsibilities, and handling problems during the absence of the manager
- Conducts in-service education lectures for medical staff, nurses, pharmacists, students, and other healthcare personnel regarding drugs, procedures, and policies
- Supports and assists pharmacist supervisors, managers, and directors to integrate departmental and organizational goals into department activities
- Provides continuous, patient-specific assessment of pharmacotherapy needs, making real-time interventions with medical and nursing staff to facilitate optimal drug therapy and other pharmacy services
- Critically evaluates and interprets medical information when providing specific information to physicians
- Assists in supervising and training pharmacy technicians, pharmacy interns, and support staff
- May precept Fellows, Residents or PharmD student rotations
- Performs related work as required
