About The Position

Help Patients Achieve Better Outcomes Through Comprehensive Medication Management We are seeking a clinically focused Ambulatory Care Pharmacist to join our pharmacy team. In this role, you will work directly with physicians and other healthcare professionals to provide comprehensive medication management for patients in ambulatory settings. As an integral member of the care team, you will help patients optimize medication therapy, improve disease management, and achieve better health outcomes. Opportunities may include working in an Anticoagulation Clinic, Medication Therapy Management (MTM) clinic, or embedded directly within a physician practice. This is an excellent opportunity for pharmacists who are passionate about direct patient care, interdisciplinary collaboration, and practicing at the top of their license. Our ambulatory pharmacists spend 100% of their time in clinical practice, helping patients navigate complex therapies and achieve their treatment goals.

Requirements

  • Bachelor of Science in Pharmacy (BSPharm) or Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
  • Licensed or eligible for licensure as a Pharmacist in Indiana and Kentucky
  • Ability to obtain required licensure within 45 days of employment
  • Strong clinical knowledge and commitment to evidence-based patient care
  • Excellent communication and relationship-building skills
  • Ability to collaborate effectively within interdisciplinary healthcare teams
  • Passion for patient education, chronic disease management, and ambulatory care services
  • Commitment to continuous learning and professional development

Nice To Haves

  • Experience in ambulatory care, clinical pharmacy, hospital pharmacy, or community pharmacy practice
  • Experience providing medication therapy management or chronic disease state management services
  • Interest in obtaining advanced ambulatory care credentials and board certification

Responsibilities

  • Collaborate with physicians and care teams to provide comprehensive medication management services
  • Evaluate medication regimens and make evidence-based recommendations to optimize patient outcomes
  • Manage patients through ambulatory care services such as Anticoagulation Clinics and Medication Therapy Management programs
  • Provide patient education, medication counseling, and chronic disease management support
  • Monitor patient response to therapy and make clinical interventions when appropriate
  • Document clinical activities and recommendations within the electronic medical record
  • Partner with patients and providers to improve medication adherence and address barriers to care
  • Ensure compliance with regulatory, accreditation, and professional practice standards
  • Participate in quality improvement initiatives and identify opportunities to enhance patient care and operational efficiency
  • Contribute to educational activities through presentations, staff development, patient education, and community outreach
  • Support the development of future pharmacy professionals through student and resident precepting opportunities
  • Provide guidance and oversight to pharmacy technicians as needed
