This role is for a full-time Pharmacist 2 in the Outpatient Central Services department at the University of Rochester. The position involves dispensing medications for patients in an outpatient pharmacy setting, overseeing compounding, preparation, and dispensing of medications in compliance with laws and regulations. The pharmacist will also answer medication-related questions, counsel patients on medication use, and supervise pharmacy technicians. The role requires understanding and following departmental and hospital policies, accurate and timely dispensing, resolving problem prescriptions, maintaining patient profiles, and directing technician workflow. The pharmacist will check all products requiring approval before dispensing and participate in quality assurance processes, including reporting medication incidents, adverse events, and errors. Additionally, the role involves developing or participating in clinical practices, interacting with healthcare professionals and patients as part of an interdisciplinary team, recommending appropriate medication use, and participating in clinical initiatives such as immunization, discharge planning, transplantation, wellness, and screenings. The pharmacist will monitor assigned patient populations, recommend therapy adjustments, document clinical activities, and participate in the education of pharmacy staff and other healthcare professionals. Problem resolution through clear and concise communication is essential, as is proactively identifying and resolving patient care problems and barriers to patient discharge. The pharmacist may also perform "charge pharmacist" duties, coordinating shift resources, problem-solving, and resolving staff conflicts. Contacting a supervisor for assistance when a solution is beyond expertise or authority is also part of the role. Other duties as assigned.

Requirements

• Bachelor's degree in Pharmacy and less than 1 year of relevant ambulatory/community pharmacy experience required
• Successful completion of a Post Graduate-Year 1 (PGY-1) accredited pharmacy practice required
• Licensed Pharmacist in New York State upon hire required

Responsibilities

• Dispenses medications for patients seen in an outpatient pharmacy.
• Oversees and directly contributes to compounding, preparation and dispensing of medications for patients in accordance with all applicable state and federal laws and regulations for Pharmacy practice.
• Answers medication related questions from patients and providers, and provides patient counseling on proper medication use.
• Supervises pharmacy technicians.
• Understands and follows departmental and hospital policy and procedures related to dispensing of medications.
• Dispenses medications accurately and in a timely manner.
• Resolves problem prescriptions independently and appropriately in a timely manner.
• Processes/validates prescriptions, utilizing current information system technology to create and maintain accurate patient profiles.
• Directs and supervises the work of technicians to ensure appropriate workflow as required.
• Communicates verbally to all hospital staff in an effective and professional manner.
• Checks all products requiring a pharmacist approval prior to dispensing.
• Participates in departmental Q/A processes, such as reporting and following up on medication incidents, reconciling medical administration records (MARs), reporting medication adverse events and errors, etc.
• Develops or participates in a clinical practice by interacting with physicians, nurses, other health care professionals, and patients as a member of an interdisciplinary team to guide and recommend appropriate medication use.
• Participates in focused clinical initiatives as defined by the department, including immunization, discharge planning, transplantation, Wellness, Screenings, etc.
• Monitors assigned patient populations for clinical progress, including renal/hepatic function, white blood count (WBC), and other applicable laboratory tests.
• Recommends adjustments to therapy as necessary.
• Documents clinical activities and interventions on a regular basis.
• Participates in education of other Pharmacy staff and other healthcare professionals.
• Identifies need for patient teaching and performs such tasks when necessary.
• Resolves problems with appropriate and direct communication to all pertinent staff in a clear and concise manner.
• Proactively identifies patient care problems and does not pass on problems to the next shift without exhausting reasonable attempts at resolution and communicating progress.
• Proactively identifies barriers to patient discharge when practicing in Hospital setting, including financial barriers that are handed off and resolved in a thorough manner.
• Solves problems that often go beyond patient-specific issues and may require participation on interdisciplinary teams or revising hospital or departmental policy, procedures or treatment guidelines.
• Performs required duties of “charge pharmacist” when scheduled, including coordination of shift resources, problem solving, staff conflict resolution, and appropriate communication from shift to shift.
• Contacts supervisor for assistance when solution is beyond expertise or authority.
• Other duties as assigned.
