About The Position

The Children's Medical Services program is seeking a highly qualified candidate to fill the full-time, salaried position of PH RN Case Manager, based in Waycross, GA. The Southeast Health District's mission is to promote and protect the health of people in Southeast Georgia, uniting with individuals, families, and communities to improve and enhance their quality of life. They have clinic locations in 16 counties in Georgia. Requirements

Associate degree in nursing from an accredited college or university.
Completion of a PHN course within one (1) year of hire.
Current Georgia Registered Professional Nurse license.
CPR certification.

Nice To Haves

Experience working with infants or young children.
A bachelor’s degree in nursing (BSN).
One year minimum experience as a registered professional nurse.
Bilingual proficiency in Spanish.

Responsibilities

Provide comprehensive support to families of children with special healthcare needs.
Develop and manage individualized care plans.
Coordinate ongoing care.
Facilitate transition planning starting at age 12.
Actively engage and retain families in the program.
Promote CMS services throughout the district.
Build and maintain relationships with community partners.
Participate in outreach activities.
Manage enrollments.
Conduct six-month and annual reviews.
Ensure continuity of care.
Review medical records.
Approve billing and documentation for other nurses.
Assist with phone inquiries as needed.
