About The Position

CarePartners is one of Ontario’s largest accredited home health care providers, offering nursing, personal support, therapy, and rehabilitation services to patients of all ages. They serve approximately 72,000 patients annually across homes, schools, clinics, workplaces, and retirement homes, operating through 24 locations and 24 nursing clinics. Personal Support Workers (PSWs) are vital members of the healthcare team, responsible for encouraging, supporting, and promoting independence for patients in home and community care settings. This involves assisting with activities of daily living under the guidance of a Personal Support Manager. CarePartners is seeking PSWs to deliver patient and family-centered care in the Waubaushene, Coldwater, and Hillsdale areas.

Requirements

  • A strong desire and commitment to making a difference in the lives of their patients
  • Exceptional interpersonal skills
  • Ability to work both independently and as part of a team
  • Availability to work non-traditional hours, such as early morning, evenings, and alternate weekends
  • A valid driver’s license (G or G2 Classification), access to a reliable vehicle, and proof of automobile insurance
  • Valid certificate as a Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or enrollment in a Nursing program
  • Current vulnerable sector check screening, a clear background check

Responsibilities

  • Assisting with daily living activities, such as personal care (rise and retire), assistance with household tasks, respite care, medication assistance, special functions
  • Promoting a safe environment by demonstrating proper body mechanics, safe transfer techniques, and home safety
  • Providing cognitive, social and/or emotional support to the patient
  • Preparing nutritious, varied and economical meals and snacks, including special diets, as determined by the care plan, using proper food preparation, storage, and hygiene practices
