About The Position

This position acts under the clinical supervision of the Home and Care Community Care Manager as an integral part of the multi-disciplinary care team to ensure optimal health to meet the needs of clients who meet the eligibility criteria for services. The PSW participates as a contributing member of the Home and Community Care Team to ensure the efficient functioning of an organization that promotes healthy and independent living.

Requirements

  • Diploma in Personal Support Worker program from an accredited college.
  • One year experience in a community health agency working with elderly and disabled.
  • Must be a team player willing to work as a contributing member of the Home and Community Care Team.
  • Excellent interpersonal skills, friendliness, and able to relate well with elders and co-workers.
  • Excellent written communication skills with proven ability to chart progress notes according to policies and procedures of the Home and Community Care Program.
  • Active listening and communication skills, friendly and courteous and possess knowledge about and sensitivity to ageing issues and challenges.
  • Decision making and problem solving skills essential.
  • Responsible and reliable, consistent, able to work with minimum supervision.
  • Successful completion of a current recognized course in First Aid and Cardiopulmonary Resuscitation CPR or Basic Cardiac Life Support (BCLS).
  • Good interpersonal skills, responsible, dependable, possess physical strength, and be respectful, resourceful, and diligent in carrying out duties.
  • Valid driver’s license and vehicle for on-the-job use.
  • Must be able to work flexible hours.
  • Sign and maintain an annual oath of confidentiality.
  • Secure and maintain certification in Privacy and Confidentiality.
  • Must provide a criminal record check prior to commencing employment.

Nice To Haves

  • Previous experience in a dietary department would be considered an asset.
  • The ability to speak Ojibway language would be considered an asset.

Responsibilities

  • Provide personal care to promote the comfort and safety, independence and mobility of the client base, such as bathing, skin care, nail and hair care.
  • Assist clients with the tasks of daily living based on the individual needs of each client who has been assessed and a determination made of the required supports.
  • Promote comfort, safety, independence and mobility.
  • Carry out the approved care plan by providing personal care and household management support to clients.
  • Ensure effective records management in accordance with the standards outlined in the case management policies and procedures manual.
  • Respond accordingly to any health concerns and/or changes in the health status of clients and notifying Care Manager immediately.
  • Provide teachings to clients and family members in the area of self-care.
  • Provide translation services when required.
  • Provide Congregate Dining Assistance by serving meals and to encourage client socialization and enjoyment of activities which enhance the physical and mental quality of life for the frail or isolated seniors.
  • Carry on conversation with clients, listen and socialize with them.
  • Create a comfortable and safe dining environment.
  • Set table, serve food and beverages, clear and clean tables.
  • Provide Meals on Wheels support by picking up prepared meals and delivering these meals to each client on route.
  • Initiate breakfast preparation as per the directions of the Cook.
  • Make snacks available to clients, as directed by the Cook.
  • Provide custodial services/assistance in the operations and maintenance of the facility, as required.
