Job Summary: The Personal Navigator Program is a public benefits access and enrollment program that serves Montgomery County and Phoenixville, PA. This role will primarily serve residents in the Pottstown, Phoenixville, and Norristown areas. The Personal Navigator is responsible for comprehensive public benefit eligibility consultations and enrollment services for Montgomery County and Phoenixville area residents. The Personal Navigator assists people seeking comprehensive government and community assistance programs by providing access and eligibility criteria, explaining and applying rules, policies, and procedures to aid the public through the benefit eligibility and benefit acquisition processes. The Personal Navigator will work closely with the Director and act as the go-to community resource to provide education and enrollment assistance to individuals and groups within Montgomery County who traditionally have had limited access and face significant barriers to accessing and maintaining public benefits, including health insurance and other benefits through the Health Insurance Marketplace (PENNIE), the Department of Human Services, and Medicare programs. Additionally, the Personal Navigator will screen, enroll, renew, and monitor benefit eligibility for all other benefits, including, but not limited to, SNAP, LIHEAP, CHIP, rent rebates, etc.

Requirements

  • Case management experience required.
  • Excellent verbal and written communication skills, with the ability to engage with diverse community members and present information concisely and effectively.
  • A genuine empathy for community needs and a compassionate approach to addressing concerns and building trust.
  • Strong problem-solving skills and the ability to find creative solutions to challenges vulnerable populations face.
  • Sensitivity to cultural diversity and an understanding of how cultural factors may impact dynamics and interactions of varying cultural backgrounds.
  • Effective organizational and time management skills.
  • Ability to work and meet deadlines independently, as well as with a team.
  • Strong networking abilities to connect with community leaders, organizations, and resources.
  • Proficiency in basic computer applications and the ability to use digital tools for communication and data collection.
  • Familiarity with Montgomery County human services and local resources. As well as community demographics and the social and economic challenges it faces.
  • Possesses a strong interest in social justice and has experience doing advocacy work for underserved populations
  • Experience in providing direct service or enrollment of individuals into benefits access programs.
  • Awareness of public health insurance programs, such as PENNIE, CHIP, Medical Assistance, and Medicare.
  • Knowledge of eligibility and qualifications for government programs and healthcare terminology.
  • Awareness of other government programs that help to alleviate poverty, such as SNAP, TANF, CHIP, rent rebates, etc.
  • The employee must be able to handle high-stress situations on a day-to-day basis.

Nice To Haves

  • An undergraduate degree in a relevant field such as social work, psychology, public health, social services, or a related discipline is preferred.
  • Master's degree preferred.
  • Bilingual Spanish with experience with immigration statuses preferred.

Responsibilities

  • Provide screening, intake, advocacy, enrollment, acquisition, renewal, and/or referral assistance to any Montgomery County or Phoenixville area residents who seek assistance through the program and are eligible for any public benefit program (SNAP, Medical Assistance, CHIP, LIHEAP, amongst others).
  • Provide enrollment assistance through the PENNIE Health Insurance Marketplace, COMPASS or Medicare.gov for uninsured children and adults to access subsidized, low-cost, and free health insurance programs. This includes completing applications, gathering required documents, and providing impartial, unbiased information about coverage and renewal options.
  • Distribute outreach materials to community members and partners to build public awareness.
  • Attend, represent, and/or present at community events to promote the agency and program.
  • Actively participate in local networking opportunities and collaboratives.
  • Strong interpersonal and customer service skills; the ability to work comfortably with clients, volunteers, and staff of diverse backgrounds; and a high level of comfort with vulnerable and underserved populations.
  • The ability to recognize cultural, language, and learning differences and be able to assist individuals in an affirming and culturally appropriate manner.
  • Must be a team player and able to cultivate good working relationships both internally as part of integrated services and externally with staff at government offices where applications are filed, for troubleshooting and advocacy with those offices.
  • Must be a collaborative problem solver, effective communicator, and embrace a diverse team environment
  • Complete all necessary annual certifications to assist with certain health care enrollment platforms. Including participation in webinars, conference calls, and in-person trainings for continued professional development regarding the Marketplace, Medicare, and other benefits
  • Must follow all federal regulations and requirements put forth by the Centers for Medicare and Medicaid Services (CMS), including, but not limited to, privacy and security standards
  • Maintains confidentiality in dealing with VNA-Community Services, Inc., its customers, vendors, clients, and patients.
  • Complies with HIPAA regulations and agency policies related to HIPAA compliance
  • Maintains active use and personal proficiency with computer and software programs specific to office communication and agency program management –Microsoft applications, G-Suite, and other applications essential to specific job requirements
  • Track all data needed for case management, tracking, reporting, and grants, including but not limited to the number of clients served by location, benefits applied for, benefits received, status, renewal dates, and relevant case notes
  • Accepts other assignments as requested or directed by VNA for work within the scope of the employee's general job description, education and/or licensure.
