Overview The care worker will provide client care as required under the supervision of the licensed nursing staff. The care worker completes assigned tasks to ensure timely, efficient and safe client care. Communication and interactions with clients and their families is an essential component of this position.

Requirements

Experience with adults of all ages
C.N.A. or C.B.R.F. certification(s)
Previous caregiver experience preferred.
C.N.A. or C.B.R.F. certification(s)
CPR certified preferred, or willingness to obtain within 3 months of hiring.
Medication administration experience, or willingness to get trained.
A familiarity with transfer equipment: gait belts, walker, Sit to Stand, and Hoyer lifts.
Must possess strong collaborative, interpersonal, organizational, multi-tasking, verbal and written communication skills, and a desire to be part of a team.
Must demonstrate on an ongoing basis the ability to develop and maintain good working relationships with co-workers and clients
Competencies needed for this position include: demonstrate a high degree of integrity and personal commitment, possess strong analytical skills, be able to work independently and manage multiple tasks at the same time.
Must demonstrate a passion to succeed, be self- motivated, dependable, committed, and a willingness to give the best client focused care.
Required to carry or lift items weighing up to 25 pounds at waist height.
Frequently required to assist in maneuvering clients weighing 100-400 pounds and occasionally required to assist in maneuvering clients weighing over 400 pounds.
Tolerate standing and walking for up to 8 hours at a time on hard and soft surfaces.
Frequently perform activities that require stooping, bending and reaching, squatting, balancing, and trunk twisting throughout the day.
Frequently able to detect and discern both loud and soft sounds such as client calls, verbal orders, call lights, and phone calls.
Frequently able to use hands and fingers to perform simple and fine motor handling tasks, such as taking vital signs, writing.
Frequently able to use different types of visual perceptions to perform activities such as moving medical equipment, evaluating clients, and reading charts.
Occasionally required to detect unusual odors that could indicate infection or other problems with a resident, or environmental issues such as smoke.
This position provides direct care to adult and geriatric clients.
Subject to frequent interruptions by staff, residents, visitors, and volunteers.
Subject occasionally to hostile and emotionally upset residents, family members, staff, and visitors.
Possess a personal presence characterized by a sense of honesty, integrity and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals and values of St Ann Center for Intergenerational Care.

Responsibilities

Performs initial and ongoing data collection on all clients under the direction of the Registered Nurse/Licensed Practical Nurse.
Implements clients’ plans of care as developed and updated by the Registered Nurse
Provides direct client care (ADLs) and reports condition and changes to RN or on call nurse.
Assures resident safety by identifying safety hazards and emergency situations and initiates corrective action as appropriate.
Assists clients to different parts of St. Ann Center as needed.
Participates in Bi-annual CPR recertification, staff meetings and other in-services as required.
Maintains neat and orderly environment in the unit.
Housekeeping as necessary to keep rooms and unit clean.
Reports any needed supplies to the supervisor or team lead.
Documents results of client’s care within established policies and procedures and scope of practice and reports any changes in client conditions to other shifts.
Prepares and serves meals, assist with feeding as necessary.
Oversees the volunteers as needed/requested.
Answers phones.
Passes meds once training is completed.
Participates in field trips when needed.
Work in other units as designated by the supervisor.
Provides other duties as assigned and requested by management.
Complies with all policies and procedures.
Assists supervisor in providing for the personal hygiene, social and recreational needs of the respite care clients.
Performs daily tasks assisting with the nutritional needs, personal care needs and ambulation as designated by the supervisor.
Gives individual attention to clients as needed.
Attends staff meetings and team huddles, makes oral or written reports identifying any client problem areas, (social, recreational, spiritual, psychological or emotional) and/or the improvement of services.
Also attends and participates in professional seminars, workshops and all staff meetings.
Report any incident or quality of care concerns to the supervisor, on call nurse or director.
Report any maintenance issues to the supervisor.
Takes on call along with other staff.
