About The Position
We are seeking a dependable and caring Personal Care Specialist (CNA/STNA) to provide hands-on care to patients in their homes while working closely with nurses, caregivers, families, and the interdisciplinary hospice team. As a Personal Care Specialist, you will help patients remain comfortable, safe, and independent in the familiar surroundings of their own homes.
Requirements
- High school diploma or GED required
- Basic computer skills and ability to use electronic documentation systems
- Valid driver's license and safe driving record
- Reliable transportation and required automobile insurance
- Ability to travel locally between patient homes
- Ability to work safely in varying weather and driving conditions
Nice To Haves
- Current Ohio STNA/CNA certification preferred
- At least one year of healthcare or direct patient-care experience preferred
- Hospice certification (CHPNA) encouraged within two years of employment
Responsibilities
- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility
- Provide compassionate, respectful, and patient-centered care
- Promote patient comfort, safety, privacy, dignity, and independence
- Observe patients for changes in condition and promptly communicate concerns to the appropriate member of the care team
- Accurately document care and services provided using electronic documentation systems
- Communicate effectively with nurses, caregivers, family members, and other members of the hospice team
- Develop trusting and supportive relationships with patients and their loved ones
- Follow each patient's individualized plan of care
- Follow hospice policies, procedures, and applicable care standards
- Provide dependable care while traveling between patient homes throughout the service area
