About The Position

This role is ideal for compassionate individuals who are committed to serving others with dignity, empathy, and respect.As a Personal Care Specialist, you will play a vital role in helping patients maintain comfort and quality of life. Working closely with families and the interdisciplinary care team, you will provide hands-on personal care while contributing valuable observations and insights gained through your direct patient interactions.

Requirements

  • High school diploma or GED required.
  • Basic computer proficiency for electronic documentation.
  • Ability to travel and drive safely in varying weather and road conditions.
  • Valid driver's license and acceptable driving record.
  • Proof of automobile insurance meeting state requirements.
  • Successful completion of a criminal background check.
  • Ability to pass a 10-panel drug screening.

Nice To Haves

  • Current Ohio Certified Nursing Assistant (CNA) credential preferred.
  • At least one year of related healthcare experience preferred.
  • Certification in Hospice and Palliative Nursing Assistance (CHPNA) is preferred. Ohio’s Hospice provides support and resources for employees interested in obtaining certification.

Responsibilities

  • Assist patients with daily personal care needs in a compassionate and respectful manner.
  • Support family members in caring for their loved ones.
  • Observe and report changes in patient conditions to the care team.
  • Maintain accurate and timely documentation of services provided.
  • Collaborate with nurses, physicians, and other hospice professionals to ensure high-quality patient care.
