About The Position

Cappella Pueblo West is looking for a compassionate and dependable Personal Care Partner II | QMAP to join our team. Our residents count on us every day, and we are seeking someone who is committed to showing up, providing excellent care, and helping older adults thrive in a culture of belonging. In this role, you will support residents with daily personal care needs, including bathing, dressing, grooming, meals, laundry, and other activities of daily living. You will also monitor resident well-being, report changes or concerns, communicate with supervisors and team members, and help ensure each resident feels safe, respected, and cared for. The Personal Care Partner II also assists with prescribed daily medication routines in accordance with state law and established procedures under the supervision of a licensed nurse. This includes reviewing medication records, following the five rights of medication administration, documenting timely and accurately, and reporting any medication-related questions or concerns. Requirements

  • Medication certification; QMAP-A/L, Med Tech, Med Aide, Nurse Delegation (per state of operation requirements)
  • Must be able to read, write and speak the English language.
  • High school or Equivalent (GED)
  • Strong self-management and organization skills
  • Strong multi-tasking abilities with the ability to handle competing deadlines, flexible and adaptable (Open to change).
  • Ability to effectively handle difficult and sensitive issues and maintain confidentiality

Nice To Haves

  • One (1) year experience in some level of elder care preferred
  • Proficient computer skills preferred
