About The Position

Virginia Home Care Services is seeking a compassionate and relationship-driven Personal Care Case Manager to support client satisfaction, communication, and long-term retention. This role focuses on building strong relationships with clients and families while ensuring high-quality service delivery. The Personal Care Case Manager serves as a primary point of contact for clients and their families. This role emphasizes client engagement, service satisfaction, issue resolution, and ongoing relationship management to promote continuity of care and long-term client retention.

Requirements

Experience in home care, healthcare, client relations, or case management preferred
Strong customer service and interpersonal communication skills
Ability to de-escalate concerns and maintain positive client relationships
Detail-oriented with strong documentation skills
Professional, compassionate, and client-focused mindset
LPN required, RN preferred

Nice To Haves

Bilingual (English/Spanish) preferred but not required

Responsibilities

Build and maintain strong relationships with clients and family members
Serve as a trusted point of contact for client concerns, questions, and feedback
Conduct regular client check-ins to assess satisfaction and care quality
Address and resolve service concerns in a timely, professional, and empathetic manner
Support client retention through proactive communication and follow-up
Coordinate with internal teams to ensure client needs and expectations are met
Document client interactions and service updates accurately
Participate in care plan discussions and service reviews as needed
Represent the company professionally in all client-facing interactions
