About The Position

The Personal Care Attendant (PCA) provides personal care services to FHCN PACE participants within the day health center. This role supports the primary care, therapy, and adult day health staff by assisting with various activities and tasks. The PCA works under the direction and supervision of the Center Manager.

Requirements

  • High school diploma or equivalent.
  • Minimum 6 months of clinical experience as a Medical Assistant, Personal Care Aide, Certified Nursing Assistant, or EMT.
  • 1 year experience with the frail or elderly population.
  • Must complete and pass the FHCN PACE approved curriculum for Personal Care Aides upon hire.
  • Ability to communicate effectively with a variety of people, including participants, families, staff, and other agencies.
  • Ability to work effectively in a team environment and independently with participants.
  • Ability to care for geriatric patients in a compassionate and patient manner.
  • Must only act within the scope of his/her authority to practice.
  • Must meet a standardized set of competencies for the specific position description established by FHCN PACE and approved regulatory agencies before working independently.
  • Must be able to conduct daily physical activities to assist frail elders in an ergonomic manner to avoid injury.
  • Must receive and carry out orders from the Interdisciplinary Team regarding personal care services and/or monitoring of participant status.
  • Must be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
  • Must pass a criminal background check.
  • Complete the competency profile prior to providing direct participant care.
  • Ability to prepare more complex documents in Microsoft Word, including creating tables, charts, graphs, and other elements.
  • Ability to use Microsoft Excel to review and compile data, including the use of formulas, functions, lookup tables, and other standard spreadsheet elements.
  • Ability to create basic presentations in Microsoft PowerPoint.
  • Currently certified in cardiopulmonary resuscitation (CPR), if CPR is not current, will be granted 30 days from the date of hire to schedule for the next available CPR certification class provided by FHCN.
  • Job duties require the compilation of information prepared in effective written form, including correspondence, reports, articles or other documentation.
  • Effectively conveys technical information to non-technical audiences.
  • While performing duties of the job, the employee is required to move about the FHCN PACE center and actively engage with the participants.
  • Employees must be able to sit for extended periods of time, as well as work at the computer for long periods.
  • Employees are required to use their hands and fingers, especially for typing on the computer and using the mouse.
  • Employees must be able to talk and listen, particularly for phone communications.
  • Employees must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the FHCN PACE members’ care.

Nice To Haves

  • An HHA or CNA license is strongly preferred.

Responsibilities

  • Provides personal care and escort assistance in accordance with individual treatment plans, including peri-care, grooming, dressing, showering, bathing, toileting, feeding, and ambulation.
  • Reports observed changes in participants’ conditions to the Center Manager and/or other center-based clinical staff.
  • Proactively communicates participant health and safety concerns to the Center Manager and/or other program staff.
  • Demonstrates safe practices utilizing proper body mechanics and complying with universal precautions.
  • Interacts in a friendly, courteous, and professional manner with participants and their families/caregivers.
  • Participates in social and recreational activities with participants as appropriate.
  • Implements interventions, as appropriate to training, as directed by rehabilitation staff, including the use of mechanical assistance for participant transfers and mobility.
  • Occasionally accompanies participants to outside clinical appointments, when necessary.
  • Assists with food service delivery and monitoring during mealtimes.
  • Completes other projects and duties as assigned.
