About The Position

myPlace Health is a PACE (Program of All Inclusive Care for the Elderly) organization backed by SCAN Group, dedicated to supporting older adults to live safely, independently, and with dignity in their own communities. Our centers are vibrant community hubs offering customized medical care, social activities, and daily support. We are seeking a mission-driven team player to make a real difference in the lives of frail and vulnerable older adults in the Compton, CA community. In this role, you will bring warmth, kindness, and compassion to every interaction, supporting participants with personal care, accompanying them to appointments, and ensuring they feel safe, seen, and supported. You will work closely with families, caregivers, and our interdisciplinary care team (IDT) to monitor participant well-being and any changes in their health.

Requirements

  • Passion for caring for older adults and supporting high-risk seniors with complex needs.
  • Experience as a Personal Care Attendant (PCA) or Certified Nursing Assistant (CNA), especially working with frail or elderly individuals.
  • CPR and First Aid certification, or willingness to complete certification within 60 days of starting.
  • Flexibility and willingness to provide care in participants' homes when needed.
  • Valid California driver’s license, dependable transportation, a clean driving record, and current auto insurance.
  • Willingness to undergo Tuberculosis screening upon hire or provide proof of a negative screening within the last year.

Nice To Haves

  • CNA or Home Health Aide (HHA) certification.
  • Bilingual in more than one language.
  • Curiosity about AI and its power to transform operations and member service.

Responsibilities

  • Help participants with everyday activities such as bathing, grooming, toileting, dressing, laundry, and meals, in the center, clinic, or occasionally in their homes.
  • Assist participants as they get in and out of transportation vehicles and accompany them on rides or to offsite medical appointments.
  • Ensure each participant receives the correct meal and assist those who need help with eating, with patience and respect.
  • Provide gentle physical assistance throughout the day using proper techniques to ensure safety and avoid injury.
  • Assist with meal delivery and create a welcoming dining experience.
  • Monitor vital signs and observe participant well-being as directed by the care team.
  • Ensure personal care supplies are available and organized.
  • Carry out care tasks and status checks based on directions from the interdisciplinary team.
  • Communicate clearly and kindly with participants, families, team members, and outside providers.
  • Be a flexible team player ready to assist wherever needed to ensure participants receive the best care.
  • Actively support the achievement of SCAN's vision and goals.
  • Other duties as assigned.
