About The Position

The Lead Personal Care Attendant (Lead PCA) provides direct resident care while supporting the Resident Service Coordinator in the daily operations of the personal care home. This role serves as a lead staff member on assigned shifts, assisting with scheduling, staff coordination, and administrative tasks to ensure efficient operations and high-quality resident care. The Lead PCA also participates in the on-call rotation (every other week) and assists in problem-solving, communication, and oversight of day-to-day activities. This position requires strong leadership, organization, and the ability to respond effectively to resident and staff needs. Other responsibilities will include maintaining a clean and homelike environment, record keeping, including medication charting, and updating support plans as required. Recognizing and responding to the needs and changes of the residents and staff will be the key to the success of this position.

  • Minimum of high school diploma or General Equivalency Diploma (GED) or active registry status on the Pennsylvania nurse aide registry required.
  • Must have successfully completed the Direct Care Staff Person Training Course and Competency Test offered through the Pennsylvania Department of Human Services.
  • Must successfully complete an approved medication administration course as required by the Pennsylvania Department of Human Services.
  • Current training in the care of residents with diabetes.
  • Must complete and pass this training annually.
  • Must attain 12 hours of work-related education annually.
  • Six months to one-year related experience preferred.
  • Current First Aid and CPR certification required.

Responsibilities

  • Serve as the lead staff person on assigned shifts, providing guidance and support to team members
  • Assist the Resident Service Coordinator with daily operations and workflow coordination
  • Help ensure adequate staffing coverage and assist with schedule coordination and adjustments, providing input regarding staffing needs, call-offs, and shift coverage
  • Support onboarding and mentoring of new staff.
  • Assist in resolving routine staff and resident concerns in a professional and timely manner
  • Promote teamwork, accountability, and adherence to facility policies and procedures that align with Chapter 2600 regulations
  • Assist with documentation oversight, ensuring accuracy, completeness, timeliness, and survey-readiness including RASPS, DME’s, and other required documentation at all times.
  • Monitor and ensure compliance with medication assistance protocols as related to the DHS required Train the Trainer medication training.
  • Communicate effectively with interdisciplinary teams to support resident outcomes
  • Participate in on-call rotation every other week
  • Respond to staffing, resident, and operational issues after hours as needed
  • Provide guidance and direction to staff during on-call periods
  • Communicate significant issues to the Resident Service Coordinator or Director of Senior Living
  • Maintain working knowledge of and ensure compliance with Pennsylvania Department of Human Services 55 Pa. Code Chapter 2600 Personal Care Home Regulations
  • Identify potential compliance concerns and promptly report and escalate issues to the Resident Service Coordinator or Director of Senior Living.
  • Assist during state inspections, surveys, and audits as directed
  • Assist residents with personal activities such as bathing, oral hygiene, dressing/undressing, and ambulation.
  • Assist with new residents’ orientation and ensure they become acclimated to the facility and others.
  • Responsible for safe movement around facility of the resident.
  • Report and document all changes in residents’ physical condition, behavior, or appearance timely to your supervisor.
  • Solve routine resident issues, promote healthy lifestyle habits, and provide general companionship and advice.
  • Documentation must be completed accurately, completely, and in a timely manner.
  • Communicate with others in the facility in an efficient and effective manner, this includes interdepartmental and interdisciplinary teams for the best possible outcomes of the residents.
  • Coordinate specific work tasks with other personnel as well as with other departments to ensure the smooth and efficient flow of information.
  • Perform regularly scheduled rounds and monitor residents’ conditions.
  • Respond to residents’ call bells promptly.
  • Monitor self-medicated residents as per current medication order and resident service plan.
  • Respond to fire alarms as required (refer to Fire Safety Manual for employees’ responsibilities and those that are specific to your position.
  • Treat all residents fairly, with kindness, dignity and respect.
  • Maintain confidentiality of all resident and facility information.
  • Report all complaints, concerns made by the resident, allegations of abuse or neglect to direct supervisor
  • Follow all safety procedures and precautions in the performance of your duties and when operating equipment.
  • Report all safety violations, unsafe or hazardous conditions and any equipment issues immediately.
  • Review Corporate Compliance educational materials, comply with Corporate Compliance directives, and report any Corporate Compliance issues according to established procedures.
  • Recognize your role as part of the QAPI efforts of the organization.
  • Attend trainings to build understand and capacity to undertake QAPI work.
  • Carry out QAPI roles and responsibilities as assigned.
  • Follow established policies and procedures in support of QAPI efforts.
