About The Position

This is a Home Care position for the Central Valley PACE program in Modesto, supporting the mission, vision, and values of Golden Valley Health Centers. The role acts as the Lead Patient Care Assistant, delivering patient care activities and tasks as assigned to assist nursing staff. The position requires travel throughout Stanislaus County and involves providing basic personal care assistance and support with activities of daily living across various settings to assigned participants. The role also includes mentoring and training other PCAs, communicating patient needs, performing tests, assisting in other departments during downtime, delivering medical equipment, and participating in home safety assessments. Effective communication with participants, families, staff, and other agencies, as well as working effectively in a team setting, are crucial. Documentation of treatments, observations, and patient activities is required, along with participation in participant care planning and teaching/learning processes. The position demands observing participants for changes in functioning and reporting them to a licensed nurse, and interacting professionally with geriatric individuals, including those with cognitive decline or physical frailties. Other projects and duties as assigned.

Requirements

• Valid California Driver’s License, acceptable driving record and vehicle insurance.
• Must have reliable transportation.
• Must be able to treat people of all ages including geriatrics.
• Meet a standardized set of competencies for the specific position description established by Central Valley PACE and approved by CMS before working independently.
• High School Diploma or equivalent required.
• Current unencumbered CA licensure as a Certified Nurse Assistant, Home Health Aide, Emergency Medical Technician, Advanced Emergency Medical Technician or Paramedic; or certification from an accredited program for clinical medical assisting.
• Current BLS CPR Card certified by the American Heart Association
• Prior personal care assistance experience, or ability to pass internal competencies within six (6) months of hire.

Responsibilities

• Acts as the mentor to PCAI.
• Communicates patient care needs to the LVN, supervising RN and update Manager.
• Assists in training PCA’s and other staff.
• Perform COVID and Influenza test when assigned.
• May assist in Day Center and Activities departments when there is down time.
• Deliver DME to PRT’s home and document in EMR.
• Assist with Home Safety Assessments when assigned only when there is down time on the schedule.
• Provides basic personal care assistance and assistance with any other activities of daily living across settings to assigned participants.
• Communicates effectively with participants, families, staff, other departments and agencies.
• Works effectively in a team setting.
• Documents treatments/procedures, observations, nursing care and patient activities on appropriate forms.
• Participates in teaching/learning process with others.
• Follows guest relations policies and procedures.
• Abides by all established policies, rules and regulations.
• Attends staff meetings and takes part in participant care planning.
• Observe each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.
• Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
• Performs patient care activities appropriately, reporting any circumstances or change in patient’s condition.
• Other projects and duties as assigned.
