FT PACE Personal Care Aide
About The Position
Care Guide Partners PACE, part of Hosparus Health, provides comprehensive medical care and support services to adults age 55 and older who need nursing-home-level care, helping them remain safely and comfortably in their homes. The Personal Care Aide (PCA) provides personal care and supportive services to PACE participants in collaboration with the Interdisciplinary Team (IDT) and in accordance with each participant’s plan of care (POC). Services are primarily delivered in participants’ homes, but may also be provided in the PACE center or other care settings, including long-term care and assisted living facilities. The PCA supports participant safety, comfort, and independence by assisting with activities of daily living, maintaining a clean and safe environment, and observing and reporting changes in participant condition to clinical staff. The schedule is Monday through Friday, 8:30 a.m. to 5 p.m.
Requirements
- High school diploma or General Education Degree (GED).
- Must be at least 18 years of age.
- At least one year of experience working with a frail or elderly population, or receive appropriate training upon hire.
- Must maintain CPR certification.
- Knowledge of safety and infection control requirements for healthcare facilities.
- Knowledge of medical equipment use and maintenance.
- Demonstrated ability to provide compassionate care and communicate effectively with adults and older adults.
- Must be a licensed driver.
Nice To Haves
- One year of home health aide experience.
- Experience using technology-based tools, including electronic medical records.
Responsibilities
- Provide personal care and supportive services to PACE participants in collaboration with the Interdisciplinary Team and according to the plan of care.
- Assist participants with activities of daily living.
- Maintain a clean and safe environment for participants.

- Deliver services primarily in participants’ homes and, as needed, in the PACE center or other care settings such as long-term care or assisted living facilities.
