About The Position

UCP Heartland is seeking compassionate and committed PERSONAL ASSISTANTS (Direct Care Providers) for FULL-TIME shifts. UCP Heartland offers a positive, team-driven, work environment. As a PERSONAL ASSISTANT/DSP (Direct Care Provider), you will perform responsibilities such as providing physical assistance with daily tasks, assisting with personal hygiene, and facilitating fun activities. You will also follow medical orders and administer medications, assist with ADL's, perform lifts and transfers (including ceiling lifts, repositioning in wheelchairs, etc.), and provide adaptive transportation. Responsibilities include shift documentation using Set-Works and general housekeeping. You will interact with family often and are expected to have a fun, outgoing, and positive personality. Most importantly, you will provide choices to the clients for their daily activities to provide entertainment and community outings. Requirements

  • High School diploma or its equivalent (GED).
  • Valid Driver's license
  • Clean Driving Record and successfully completes and maintains assigned drivers training
  • A MINIMUM OF ONE WEEKEND SHIFT PER WEEKEND
  • Experience supporting individuals with disabilities or in a customer service field
  • Ability to take direction and work with minimal supervision.
  • Ability to consistently work in a professional, cooperative manner with all Agency clients and personnel.
  • Good written and oral communication skills
  • Computer Proficiency
  • Reliable transportation
  • Must possess a valid driver's license and a good driving record.

Responsibilities
