UCP Oregon is seeking two Personal Agents (Caseload Managers) to work in their intellectual/developmental disabilities support services brokerages, UCP Connections and Mentors. This role involves working with 36 to 40 adults on the caseload, who live in Multnomah and Clackamas Counties and represent diverse backgrounds, disabilities, and living situations. The Personal Agent will assist customers and their families in navigating the support services system, using person-centered planning to assess needs and develop plans, securing resources and services (employment, housing, crisis, direct support), and addressing health and safety needs. The role requires advocating for clients, providing care-coordinated problem-solving (sometimes urgently), managing intensive paperwork (plan development, billing, progress notes, caseload files, correspondence), monitoring service quality, and ensuring client choice and independence.
