About The Position

Arms Wide is seeking a Permanency Support Services Case Manager to join their team. This role is a combination of Post Permanency and Post Adoption Case Management, focusing on providing information and referral services to post-adoption and post-permanency families. The Case Manager will develop and implement community outreach to raise awareness of available services and maintain effective working relationships with other Child Placing Agencies to ensure a continuum of care for families who have adopted from foster care or have Permanent Managing Conservatorship (PMC) of children. The primary goal is to strengthen family functioning, improve quality of life, promote safety and security, help families cope with a child's history of abuse, and prevent the long-term removal of children from the home, ultimately aiming to keep families intact.

Requirements

  • Bachelor’s Degree in Social Work or related area.
  • At least one year of child welfare or child placing experience.
  • Bilingual applicants required.
  • Basic knowledge of human growth and development.
  • Understanding of family dynamics theory and practice.
  • Good, basic, clerical and administrative skills.
  • Emphasis on verbal and written communication skills.
  • Positive attitude.
  • Willingness to take initiative.
  • Ability to maintain confidentiality.
  • Ability to use a computer, computer software, scanner, printer, telephone, copier, calculator, and postage machine.
  • Reliable transportation required.

Responsibilities

  • Provide information and referral services to post adoption and/or post permanency families seeking services.
  • Develop and provide outreach to the community to increase awareness of services available to families.
  • Maintain an effective working relationship between all Arms Wide programs and other Child Placing Agencies.
  • Plan and coordinate post adoption or post permanency services to a child and their family.
  • Assist families in promoting safety and security within the family.
  • Help families cope with any history of abuse in the child's background.
  • Work to avoid permanent or long-term removal of children from the family.
  • Help the family remain intact.
