Requirements

• Bachelor's degree in social work, counseling, or a related field
• Experience working with trauma-impacted individuals and families
• Strong relationship-building and one-on-one communication skills
• Excellent organizational and computer skills
• Flexibility to work evenings, weekends, or non-traditional hours as needed
• Ability to adapt in a dynamic, community-based environment

• 1–2 years of related experience and/or internship

Responsibilities

• Work closely with children, families, and referral sources to build and execute comprehensive permanency plans.
• Connect adoptive and kinship families with the individualized support they need for stable, lasting placements.
• Provide the full range of SWAN service units, including: Child Profiles and Child Preparation Services, Child-Specific Recruitment, Family Profiles and Placement, Finalization support.
• Conduct in-home visits, case advocacy, and ongoing family support.
