This role supports high-risk mothers and families identified for navigation during pregnancy and transition to delivery or NICU admission. The Perinatal Nurse Navigator facilitates communication among various medical teams, ensures seamless transitions across care settings, and collaborates with multidisciplinary teams. The position also involves interfacing with external organizations and leading Perinatal Rounds to promote communication and proactive care planning for high-risk patients.

Requirements

Bachelor’s degree in Nursing required
NRP – Neonatal Resuscitation Program upon hire required
BCLS – Basic Cardiac Life Support upon hire required

Nice To Haves

Master’s degree in Nursing preferred
Three years of Level III NICU experience preferred

Responsibilities

Supports high risk mothers and families identified for navigation during pregnancy and transition to delivery or NICU admission
Facilitates communication among maternal-fetal medicine, obstetrics, neonatology, pediatric subspecialists, palliative care, outpatient providers, labor and delivery, and NICU teams
Ensures seamless transitions across inpatient, outpatient, and community based care settings
Collaborates with obstetric, neonatal, surgical, nursing, social work, case management, chaplaincy, and therapy teams
Interface with external organizations for documents needed for the coordination of activities, programs and partnerships
Prepares and leads bi monthly Perinatal Rounds to promote communication, coordination, and proactive care planning for high risk antenatal and fetal patients
