About The Position

This position is part of a home visiting program focused on perinatal health in Bryan, Camden, and Long County. The Georgia Department of Public Health is the lead agency dedicated to protecting lives and promoting healthy lifestyles across the state. Requirements

  • Associate degree in nursing from an accredited college or university.
  • Completion of a PHN course within one (1) year of hire.
  • Current Georgia Registered Professional Nurse license.
  • CPR certification.
  • Valid driver's license.

Nice To Haves

  • Previous experience in home care and/or medical case management.
  • Experience with Newborn and/or Pediatric populations.
  • Bilingual in Spanish.
  • One year of Labor & Delivery experience.
  • One year working in a prenatal care clinic.
  • One year experience working with moms and babies.

Responsibilities

  • Conduct home visitation services for medically diagnosed high-risk pregnant, post-partum, and inter-conceptual women.
  • Provide nurse case management, including referrals for support, education, and community resources.
  • Conduct outreach to target populations to reduce disparities in birth outcomes.
  • Provide general clinical support in the home.
  • Communicate with healthcare providers regarding client status updates throughout the program.
