About The Position

To provide medical case management services to the obstetric patients of Pueblo Community Health Center, Inc. This role involves providing routine prenatal care, medical screening, patient education, and follow-up for obstetric patients. The Perinatal Case Manager will also provide care for gynecological patients and assist in managing high-risk obstetrics (HROB) cases. The position requires frequent travel for patient care, business meetings, and community events, potentially including evenings and weekends. The role also involves maintaining patient records, ensuring confidentiality, and staying updated on current perinatal nursing technology.

Requirements

  • Registered Nurse with valid Colorado license
  • Additional training in health education
  • Valid driver’s license and auto insurance as required by law
  • Current B.L.S. with skills demonstration
  • C.P.R. certification for Healthcare Providers required
  • Ability to work independently

Responsibilities

  • Perform initial OB screening
  • Schedule OB visits
  • Provide patient education
  • Follow up on no shows and lab results
  • Maintain OB patient charts
  • Perform triage of OB phone calls
  • Transfer information to Labor & Delivery
  • Perform daily hospital rounds
  • Perform routine point of care testing in the lab
  • Assist in keeping the urine lab clean and orderly
  • Maintain inventory of exam room supplies, stock and order supplies as needed
  • Provide patient education including quarterly Lamaze class series and one-to-one prematurity prevention counseling sessions
  • Maintain monthly statistics on the OB population
  • Assist in scheduling HROB, case managing HROB visits and assisting in HROB clinic
  • Assist in maintaining linkages with community agencies
  • Ensure that all points of contact with the patient are adequately documented in the medical record, including patient phone calls, visits, and contact with other community agencies
  • Maintain patient confidentiality
  • Provide gynecological and perinatal care appropriate to the age-specific needs of all patients
  • Maintain knowledge of current technology related to perinatal nursing
  • Assist with gynecologic procedures and case management of gynecology patients
  • Participate in the peer review process in the department
  • Provide services to an individual who is not a patient of the health center in emergency situations
  • Provide supervision of students and medical residents providing care at the health center’s facilities
  • Perform other duties assigned
