The primary function of the Performance Improvement Specialist is to support and facilitate quality assessment and performance improvement (QAPI) activities for the local hospice branch. The QAPI Program is an on-going, data driven, organization-wide program involving data collection, analysis, and collaborative development of performance improvement initiatives. This role involves performing quality monitoring activities, including tracking, trending, and analyzing data associated with various reporting programs like Hospice Quality Reporting Program (HQRP) and Care Compare. The specialist will also manage CAHPS analysis, service comments, infection control reporting, incident reporting, and root cause analysis. Additionally, the role includes revenue analysis, tracking Hospice Aide in-service hours, and ensuring compliance with HIPAA/OSHA annual updates. Physician satisfaction surveys, revocation and discharge monitoring are also part of the responsibilities. The specialist will collect and analyze quality indicator data/key performance indicators, collaborate with senior management to identify improvement priorities, and develop initiatives. Regular quality reporting updates, facilitation of Performance Improvement Projects, and maintenance of documentation are key aspects of the role. The specialist will also identify program needs based on data analysis, recommend training, and report on compliance with required trainings. Acting as a liaison to Central Support for external audits and participating in pilot projects are also included. For programs with an Assistant, oversight and mentorship are expected.
