About The Position
Stella Maris Home Health has an opening for a Per Visit-Admissions Nurse (RN) to perform home hospice start of care visits. This position will cover clients on a per visit basis in territories throughout Baltimore County/City. Home Hospice experience is required.
Requirements
- RN licensure in the State of Maryland
- Experience in Epic and Oasis required
- Prior experience in home health/hospice required
- Valid driver’s license and reliable transportation
Responsibilities
- Under the direction of the clinical manager, the Home Hospice RN performs primary nursing care functions in conformance with professional standards of hospice nursing practice.
- Providing individualized care to patients in their home under the plan of care approved by the primary physician.
Benefits
- Flexible Schedule-Pay is “Per Visit”
- Per visit rate is $60.00 ($80.00 for weekend)
- Start of Care visit is $175.00 ($250.00 for weekend)
- Additional rates for specialty visits
