About The Position

Experienced Wound Care Registered Nurse needed for per diem home care visits in Chicago. We've seen a sharp increase in complex wounds in our home care clients. Looking for a nurse to assist with the increased number of visits as well as complexity and help provide education for clients, their families, and other nurses. Requirements

Current and unrestricted RN license in state for which you are applying (Illinois).
Minimum 2 Years of Experience with Wound Cares of all complexities.
Excellent Documentation and Communication Skills
Proof of eligibility to work in the US.
Reliable transportation to downtown Chicago
Clean background and criminal record.
Cleared TB Test
CPR Certification
Cleared Physical to Work

Nice To Haves

Home Health or Home Care Experience preferred.

Responsibilities

Perform care as defined by client’s individual Plan of Care and within scope of state’s nurse practice act.
Monitor patient’s condition and notify appropriate personnel of any changes.
Provide those services requiring substantial and specialized nursing skill.
Assures proper maintenance of clinical records in compliance with local, state and federal laws.
Counsels the client and family in meeting nursing and related needs.
