The Per Diem RN is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit. This role involves completing clinical admissions, explaining services, educating patients/families, obtaining informed consents and initial assessments, providing information to physicians, initiating skilled nursing interventions, obtaining physician orders, and completing documentation. The RN also acts as the Company representative at assigned facilities, facilitates referrals, communicates with Admissions, provides clinical communication, obtains consents, arranges safe discharges, obtains medical record information, develops relationships with case managers and physicians, educates on services, and provides care for GIP patients in hospitals. Responsibilities include managing direct patient care, coordinating the Plan of Care, educating patients/families, reporting changes in patient condition, evaluating referrals, attending patient deaths, providing bereavement interventions, presenting reports to the IDG, communicating with physicians and staff, and supervising other nursing team members. During emergencies, the RN may be required to report to a designated company location to ensure continuity of services.
