This is a per diem Wound Care Center Coordinator position at Vassar Brothers Medical Center in Poughkeepsie, NY. Under the direction of the Nurse Manager/Program Director, the Wound Care Coordinator communicates and collaborates closely with physicians, other WCC staff, and other members of the health care team, to provide consistent quality care for patients with wounds. Working with a high degree of autonomy, the Coordinator accepts responsibility and accountability for assigned patients as a case manager to plan, implement, and evaluate the care of the patients.

Requirements

  • Requires Bachelors degree with a major in nursing (BSN).
  • Graduate of accredited School of Nursing
  • Current NYS RN license.
  • RN
  • BLS

Nice To Haves

  • Wound Care Certification Preferred
  • Three (3) years of experience as staff nurse in a medical/surgical or home care setting preferred.
  • Wound Care experience preferred.

Responsibilities

  • Provides patient care that is continuously evaluated, planned for and revised as needed.
  • Plan patient care to reflect goals that are realistic, measurable, and consistent with the medical plan of care for each patient, to include discharge planning.
  • Organize and prioritize work to provide health information, discharge planning, and patient care in a safe environment to maximize health in the continuum of care and protect the patient.
  • Demonstrates the responsibilities as a professional nurse and case manager through accurate, clear, concise, and timely written and oral communications.
  • Demonstrates knowledge and professional utilization of current trends and practices in the health care field.
  • Utilizes material, equipment and time in a safe, therapeutic, and cost effective manner.
  • Participates in Quality Assurance activities.
  • Demonstrates regard for the dignity and respect of all patients, family members, visitors and medical center personnel as defined in the philosophy of Vassar Brothers Medical Center.
  • Maintain and Model Nuvance Health Foundational Values (Personal, Imaginative, Agile, Connected).
  • Performs other duties as required.
