Requirements
- 5+ years of RN experience in Rehab, Long-Term Care, or Assisted Living.
- 5+ years of experience in client care planning and management.
- Active Rhode Island RN License.
- CPR Certification.
- Valid driver's license.
- Insured vehicle.
- Ability to rapidly assess symptoms and make high-stakes decisions in a home environment.
Responsibilities
- Initiate and manage new cases.
- Conduct home evaluations.
- Provide clinical supervision for CNAs.
- Administer physician-prescribed medications.
- Perform advanced wound care.
- Manage medical devices.
- Accompany and advocate for clients at physician appointments.
- Lead hospital discharge planning.
- Provide teaching to patients and families.
- Utilize EMR software to communicate clinical updates.
