Requirements

• 5+ years of RN experience in Rehab, Long-Term Care, or Assisted Living.
• 5+ years of experience in client care planning and management.
• Active Rhode Island RN License.
• CPR Certification.
• Valid driver's license.
• Insured vehicle.
• Ability to lift 50 lbs.
• Ability to assist with transfers.
• Ability to travel across Providence, Kent, and Washington Counties.

Responsibilities

• Initiate and manage new cases.
• Conduct home evaluations.
• Provide clinical supervision for CNAs.
• Administer physician-prescribed medications.
• Perform advanced wound care.
• Manage medical devices.
• Accompany and advocate for clients at physician appointments.
• Lead hospital discharge planning.
• Provide teaching to patients and families.
• Utilize EMR software to communicate clinical updates.
