About The Position

Circadia Health is seeking a Per Diem Nurse Practitioner to join their team in Orange County. This role involves transforming bedside care with AI by acting on predictive health signals in real-time and coaching facility teams to reduce preventable hospitalizations. The NP will blend direct patient care with technology integration through training, live demos, and shadowed assessments. The position focuses on preventing avoidable hospitalizations and improving senior-care operations using Circadia's AI platform, which combines contactless sensing, predictive models, and enterprise integrations.

Requirements

• Certified and licensed Nurse Practitioner in CA.
• ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation.
• 2+ years NP experience with complex adult populations.
• Able to practice independently.
• Valid CA driver's license.
• Comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required.

Nice To Haves

• Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
• Spanish is preferred.

Responsibilities

• Conduct initial and follow-up assessments on flagged residents.
• Initiate/adjust plans of care.
• Complete consent forms.
• Coordinate with facility providers.
• Train and coach staff at the point of care, standardizing the use of Circadia into clinical workflows.
• Interpret trend-based insights.
• Prioritize high-risk patients.
• Drive timely interventions to prevent decline.
• Complete documentation/charts within the same day.
• Complete visits across designated SNFs (typically 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
• Surface field insights to improve workflows and tech integration in a clinical setting.
• Support NP onboarding, share best practices, and partner with clinical ops/product teams.
• Cover sites within a 30-40 mile radius; mileage and lodging covered per policy for sites outside this radius.
