About The Position

We are seeking experienced and compassionate Licensed Practical Nurses (LPNs) to join our Manchester-area home care team on a per diem basis. This position offers independence, flexibility, and meaningful one-on-one care for patients who prefer to remain in the comfort of their homes. Position Overview The LPN will provide skilled nursing visits focused on promoting recovery, maintaining stability, and enhancing quality of life. Requirements

• Current and active LPN license in the State of New Hampshire (required; not a compact state).
• Valid driver’s license and reliable transportation.

Nice To Haves

• Prior experience in home care, long-term care, or rehabilitation preferred.
• Strong communication and documentation skills.

Responsibilities

• Conduct in-home patient assessments, medication management, and wound care.
• Monitor and document changes in patient condition and report to supervising RNs or physicians.
• Educate patients and caregivers on treatment plans, medications, and home safety practices.
• Maintain accurate, timely documentation using electronic charting.
• Collaborate with multidisciplinary care teams to ensure coordinated and effective patient care.
