About The Position

Helio Health is seeking a Peer Case Manager to support its mission of promoting recovery from substance use and mental health disorders. This role is crucial for managing and coordinating services for individuals in low-barrier OMH homeless scattered site supportive housing units. The case manager will assist individuals in maintaining housing, navigating health and human services, and developing skills. This position requires providing in-community, in-person support at least 4 times per month upon program entry. The Peer Case Manager will collaborate with the individual and collateral supports to develop and execute an Individual Service Plan (ISP) with goals focused on housing retention, community integration, and recovery, with quarterly reviews and updates. This is a Full-Time position located at the Supportive Housing First Rochester Program. The pay rate is $20.00 - $24.00 per hour. Full-Time hires are eligible for a Sign-On Incentive up to $1,500.

Requirements

  • High school diploma required.
  • Individuals with lived experience preferred.
  • Lived experience with homelessness, behavioral health and/or substance abuse issues.
  • Valid NYS Driver’s License preferred.

Nice To Haves

  • Associate's degree in a related human services field preferred.

Responsibilities

  • Maintains a caseload and performs primary casework responsibilities including intake, orientation, referrals, counseling, crisis intervention, patient/resident education, case management, reporting and record keeping, consultation with other professionals, coordination of care with other community providers, and discharge planning.
  • Performs Quality Assurance activities.
  • Provides services according to agency policies, procedures, program guidelines, and governmental regulations.
  • Monitors provision of services when assigned.
  • Evaluates programs and identifies strengths/weaknesses.
  • Assists in problem resolution and implementation of supervisor approved program modifications.
  • Participates in program administrative and clinical committees as assigned.
  • Represents agency to the community.
  • Other duties as assigned by supervisor.
