Trillium Health Resources is seeking a Peer Support Specialist to join their Support Transition and Reentry (STAR) team. The STAR program serves adults with behavioral health or intellectual/developmental disabilities who are exiting incarceration, providing care coordination pre- and post-release. The Peer Support Specialist will educate members and guardians on navigating various systems, identify community-based services, and work with the recipient's care team to establish transition linkages for school, employment, housing, community service opportunities, and other supports. This role involves addressing reentry barriers such as access to insurance, disability benefits, transportation, food, clothing, and housing. The position will also establish relationships with justice systems, including jails, prisons, Probation and Parole, and Pre-release officers.
