The Forensic Assertive Community Treatment (FACT) program is an evidence-based, interdisciplinary team model serving individuals with severe mental illness, often with co-occurring substance use disorders and justice-system involvement. FACT provides intensive, community-based services focused on promoting stability, recovery, and successful community integration through wraparound care coordination, psychiatric services, substance use treatment, peer support, and collaboration with judicial and community partners. As a Peer Recovery Coach II – FACT, you will be an essential member of this multidisciplinary team, using your lived experience to build authentic, supportive relationships that promote hope, self-determination, and long-term recovery in community and field-based settings.
